What you'll accomplish
Record a 60-second voice memo after any provider call, peer-to-peer discussion, or physician advisor conversation, and get a complete text case note without touching a keyboard. The result is more complete documentation than manual note-taking, saving 10–15 minutes of typing per call.

What you should see: Real-time transcription appearing on screen as you speak test words.
The key to useful transcription is a consistent format. After any significant call, record a 60–90 second voice memo following this structure:
"Case note for [de-identified case ID or initials]. I just spoke with [caller role: provider, payer, physician advisor]. Date: [today]. Key points: [one to three sentences about the discussion outcome]. Decision: [what was decided or what action is needed]. Follow-up needed: [yes/no, and if yes, what and when]."
This structure becomes automatic after a week of practice.
What you should see: A transcribed note with the structure you spoke, with all the key information captured without touching a keyboard.
Open the transcribed note in Otter.ai. Medical terminology sometimes gets garbled. Common errors to watch for:
The cleanup takes 1–2 minutes and is still faster than writing the note from scratch.
Copy the cleaned transcription and paste it into the case notes field of your UM platform. Add any case-specific identifiers (auth number, member ID) that you didn't include in the memo for HIPAA compliance.
What you should see: A complete case note in your UM system with more detail than you'd typically capture from memory.
The biggest challenge with this workflow is remembering to record immediately after the call. Set a practice rule: before you dial your next call, record the memo from the previous one. The 90-second recording prevents the common "I'll write it up later" problem that leads to incomplete documentation.
Post-Provider Call Note:
"Case note. Provider call. Spoke with [provider type]. Purpose of call: [reason]. Provider's response: [summary]. Information provided by provider: [key facts]. Action needed: [what happens next]. Timeframe: [deadline]."
Post-Payer Authorization Call:
"Case note. Payer authorization call. Spoke with [payer name]. Auth status: [approved/denied/pending]. Details: [specific decision, dates, conditions, limitations]. Follow-up action: [what UR needs to do next]. Reference number: [if provided]."
